Dr. Ravish Narvel MD, FACP, CMD

Medical Director

Dr. Narvel is an esteemed physician with over three decades of experience in internal medicine. Board certified by the American Board of Internal Medicine, he has proven himself as a dedicated leader. His extensive expertise spans various subspecialties, including geriatric medicine, cardiology, and pulmonology. Dr. Narvel is deeply committed to medical education, mentoring students from Lecom Medical School, St. Vincent’s Family Practice Residency Program, and more. With a passion for research, Dr. Narvel has led impactful studies on topics such as bronchogenic carcinoma and proton pump inhibitors.

Dr. Abdallah Kafrouni MD

Pulmonary Program

Dr. Kafrouni is a pulmonologist who specializes in lung diseases and sleep disorders and provides comprehensive pulmonary care to all of his patients. He is certified by the American Board of Sleep, American Board of Internal Medicine – Critical Care Medicine, and American Board of Internal Medicine – Pulmonary Disease. He is affiliated with the four Ascension St. Vincent’s Hospitals in Jacksonville, Florida. He is a graduate of American University of Beirut and completed his fellowship at University of Louisville Hospital and his residency at Medstar Good Samaritan Hospital.

Dr. Junaid Ahmed MD, FACC

Cardiac Program

Dr. Ahmed is the president and founder of Cardiac Vision, LLC. His education includes a General Cardiology Fellowship at the University of Florida at Shands Jacksonville, an Internal Medicine Residency at Loyola University Medical Center, and a Doctor of Medicine from Creighton University School of Medicine. He holds COCATS certifications in various cardiology specialties, including ECHO, Nuclear Cardiology, and Cardiac CT. His transitional cardiology practice focuses on providing comprehensive cardiac care to patients at rehabilitation or LTAC facilities, aiming to increase functional capacity, optimize cardiac medications, and reduce hospital readmissions. He is fluent in English, Hindi and Spanish.

Dr. Carlos Zamora MD

Cardiac Program

Dr. Zamora offers comprehensive cardiovascular care, including echocardiograms, vascular ultrasounds, EKGs, and stress tests at Optimum CV, PLLC. He previously served as the medical director of the Echocardiography Laboratory at Baptist Medical Center Jacksonville and led the Echocardiography Quality Program at Baptist Health. His illustrious career includes roles as a cardiologist at Baptist Heart Specialists and as the Chief Medical Officer of Vital Health, a telemedicine company. He holds certifications in Cardiovascular Disease, Nuclear Cardiology, Cardiovascular Computed Tomography, and Echocardiography. Dr. Zamora is a Fellow of the American College of Cardiology and has received numerous honors for his contributions to cardiology research and education. He is fluent in Spanish, Portuguese and English.

Dr. Erin T. Wolff MD

Physiatry

Dr. Wolff is certified by the American Board of Physical Medicine & Rehabilitation, and has a subspecialty in Sports Medicine. He lectures frequentlyand his research has been published in the book “Evidence-Based Management of Low Back Pain” and various medical journals.

Dr. Wolff is a graduate of Wayne State University School of Medicine. He completed his Physical Medicine & Rehabilitation Residency at Carolinas Medical Center and his Sports & Spine Fellowship at Atlantic Health System/ Mountainside Hospital New Jersey Sports Medicine Institute.

Dr. Rehan Shah MD

Nephrology

Dr. Shah is a board certified nephrologist who is currently at Tift Regional Medical Center. He does inpatient consultations and manages the care of patients with chronic hemodialysis and peritoneal dialysis. He received his Doctor of Medicine from the American University of the Caribbean, and completed both his Internal Medicine Residency and Nephrology Fellowship at SUNY Downstate Medical Center. Dr. Shah is a member of the American Society of Nephrology, Renal Physicians Association and American Medical Association. He has lectured for the Critical Care symposium at Shore Medical Center and has conducted public education seminars on Hypertension and Chronic Kidney Disease.

Dr. Dalia Elramady MD/MPH

Dr. Dalia Elramady is an experienced physician specializing in post-acute care and primary care. She is currently a hospitalist with HNI Healthcare in Jacksonville, Florida. With a background in family medicine and a Master’s Degree in Public Health, Dr. Elramady is dedicated to providing high-quality medical care and promoting community health and wellness.

Jesse McDonald PA-C, RRT

Jesse McDonald is a versatile healthcare professional with experience in respiratory therapy and physician assistant practice. He holds a Master of Medical Science in Physician Assistant Studies from Nova Southeastern University and has worked in various clinical settings, including internal medicine, family medicine, emergency medicine, and critical care cardiology. With certifications in BLS, ACLS, PALS, and Registered Respiratory Therapy, Jesse is dedicated to providing high-quality patient care and is a valuable addition to our team.

Christopher Basco MSN, APRN, AGACNP-BC

Christopher is a board certified Adult-Gerontology Acute Care Nurse Practitioner with ten years’ experience as a registered nurse in critical care. He is a graduate of Jacksonville University and Florida State College, and has worked in the ICU at St. Vincent’s Medical Center Riverside and at the Mayo Clinic. Christopher is ACLS and BLS certified by the American Heart Association.

Stephanie Myers, APRN

Stephanie is a board certified Adult-Gerontology Acute Care Nurse Practitioner who is ACLS and BLS certified by the American Heart Association. She is a graduate of University of South Alabama, University of North Florida and Florida State College.

Mathew Lenner, APRN

Mathew is a board certified Family Nurse Practitioner who is skilled in critical care, utilization review, emergency nursing, and corporate communications. He is a graduate of South University, Jacksonville University and Southern Connecticut State University. Mathew is BLS certified by the American Heart Association.

Ioana Lustrea MSN, APRN, FNP-BC

Ioana is a board certified Family Nurse Practitioner with 18 years of nursing experience in case management, leadership, and direct patient care. She is a graduate of Jacksonville University, Western Governors University and Miami Dade College. Ioana is BLS certified by the American Heart Association.
